SofClinic — AI Diagnostic Assist & Predictive Analytics

The Vision

The AI module inside SofClinic helps doctors surface diagnostic patterns, flag at-risk patients, and generate evidence-based treatment suggestions — all integrated seamlessly into the existing clinical workflow.

The Challenge

Healthcare providers needed actionable AI insights at the point of care without disrupting clinical workflows. Generic AI tools required expensive integration projects and dedicated data science teams to maintain, making them impractical for most clinics.

The Solution

We built a LangChain-powered diagnostic assist layer into SofClinic that analyzes patient history, lab results, and presenting symptoms to surface relevant clinical alerts. A predictive re-admission risk engine flags at-risk patients before they deteriorate, allowing proactive intervention. All AI features are embedded directly into the doctor dashboard — no extra login, no context switch.

The AI flags are genuinely useful — not noise. It caught three at-risk diabetic patients our team would have missed during an exceptionally busy week. This is AI that actually helps.
